Format for filing complaint in police station on perjury by wife in court

Dear learned members,

I had filed divorce petition and there after wife filed maintenance by providing false information as she continuing as house wife with no income though she was working.
I have requested her employer to share the details via RTI act and they declined to furnish the information.
Requested court to take action on her as she committed Perjury through my lawyer and the petition is not marked or sent to concerned police station.
And, Judge has passed verdict by dismissing my divorce petition and Judge has not considered perjury application.
Appealed/file petition in high court and due to this pandemic, the case is pending.
Meanwhile, i am planning to file a complaint in local police station on wife for she committing perjury.

I would request your help here any format and or suggestions on filing the complaint in police station against her perjury will help me alot. its been 8 years im going through mental and physical illness which my family too getting effected.
