Code of Civil Procedure – Appendix C – Form No. 2 Interrogatories (Order 11, rule 4)

 

Interrogatories propounded on the Petitioner by the Respondent

 

Pursuant of Code of Civil Procedure, the Respondent, , submits the following interrogatories to be answered by the above named Petitioner, , under oath.

 

The Petitioner shall set forth the answer to the interrogatory “separately and fully

in writing under oath” or “shall state fully the grounds for refusal to answer any interrogatory”. The response shall be signed by the Petitioner.

 

Interrogatories

 

  1. State your full name, date of birth, current telephone number, and current mailing

 

  1. Describe your educational background. Include in your answer the highest grade/ qualification you completed; the name and date of any degree,

 

diploma, or certificate you received, and the name of the institution conferring the degree, diploma, or certificate; and any specialized training you have received.

 

  1. If you’re currently employed in any capacity, identify your employer and state

  1. Job Title
  2. Your duties
  3. The number of hours in your average work week

 

  1. For each employment that you have had during the past seven years other than any current employment, identify each employer and for each employment state:
    1. The dates of employment
    2. Your duties
    3. Your wages
    4. Your reason for leaving the

 

  1. If you were unemployed for any period of time after you separated from your spouse, specify the amount and source of any income that you received while unemployed.

 

  1. Itemize the date, nature, and amounts any persons have contributed, whether directly or indirectly, to payment of any indebtedness or living expense of yours since you separated from your

Please list the dowry articles with their date of purchase, location of purchase & value of the article purchased which were given to Respondent and his family members “during” the marriage of the Petitioner and Respondent. A sufficient answer to this question must also include the name, address and telephone number of the donor as well as the date of the transaction on which article was handed over to the Respondent and his family

    2. The contact information of the witness, including the business name (if applicable), title or position held (if applicable), mailing address, and telephone number;
    3. The educational and occupational background of the witness;

 

  1. What is the relationship between you and the witness; and
  2. The general subject upon which the witness will

 

  1. Please list the date, time and location of each instance of maltreatment/ misbehavior/ physical abuse/ domestic violence that the Petitioner alleges the Respondent and his family members inflicted upon her. For each such instance, please list the following:
    1. A complete description of each instance of maltreatment/misbehavior/physical abuse/domestic violence, including the date, time and locations of the incidents of abuse or violence;
    2. When and to whom the Plaintiff first reported each instance of physical abuse or domestic violence;
    3. Whether the Plaintiff was injured by the incident, and, if so, where she sought medical treatment;
    4. Whether the police were called to the scene of the incident and, if so, whether an incident or police report was created;
    5. Whether the Plaintiff did anything to instigate the incident and, if so, what she did to instigate the incident;
    6. Whether the Plaintiff was under the influence of any drugs, alcohol or prescription medications at the time of each incident and, if so, what substance(s) she was under the influence of at the time of the incident;
    7. The name, address and work and home phone number of each witness to the
